The following agencies receive grants from AOASCC through Federal Older Americans Act funding to provide services for individuals 60 years of age or older.

Community Support Services

CHCI Mericare Senior Dental Program   
Ansonia, Bethany, Branford, Derby, East Haven, Guilford, Hamden, Madison, Meriden, Milford, New Haven, North Branford, North Haven, Orange, Oxford, Seymour, Shelton, Wallingford, West Haven, Woodbridge
CHCI utilizes Title III funding to ensure that the seniors in the communities it serves have access to high quality dental care.

Bridges Healthcare Inc., Case Management for Older Populations
Milford, Orange and West Haven
We provide mental health services services that are person-centered, dynamic and recovery-oriented to empower individuals through education and advocacy to lead healthy, meaningful and productive lives.
           
Clifford Beers, Case Management.

Clifford Beers' provides case management services to connect elderly and elderly grandparents raising grandchildren to essential services and support groups.
           
BHcare, Inc., Elder Abuse Education Services, The Umbrella Center for Domestic
Ansonia, Bethany, Branford, Derby, East Haven, Guilford, Hamden, Madison, Milford, New Haven, North Haven, Orange, Oxford, Seymour, Shelton, West Haven and Woodbridge
Provides the CHERISH (Caring Home for Elderly Receiving Insight, Safety and Support) program to fight the problem of elder abuse through community education and awareness along with support groups.

Transportation

 IVCG provides transportation services for medical appointments Monday - Friday.

Meriden Senior Center, Medical Transportation          
Meriden and Wallingford
Transportation will be provided for Meriden and Wallingford residents for out of town appointments.

TEAM, Inc, Volunteer Caregivers 
 Ansonia, Derby, Oxford, Seymour, Shelton,
Services include: Friendly visiting, shopping for clients, telephone reassurance, transportation and escort to medical appointments and errands. Handy work and yard work, snow shoveling and sanding and mail correspondence assistance.

TEAM, Inc., Medical Transportation 
Ansonia, Derby, Seymour, Shelton
Provides access to transportation for medical appointments.  Curb to curb assistance is offered for those elders who need assistance

The Mary Wade Home
Mary Wade Home's Medical Transportation
East Haven, Hamden, New Haven, North Haven and West Haven
Provides low and extremely low-income individuals ages 60+ in the greater New Haven metropolitan area with reliable and safe medical transportation.

Adult Day Center

Clelian Adult Day Center
Ansonia, Bethany, East Haven,  Guilford, Hamden, Milford, New Haven  North Haven, Orange,  Wallingford, West Haven and Woodbridge
Clelian Center members receive assistance with activities of daily living from our professional and compassionate nursing staff, promoting a maximum level of functioning and independence. Clelian Center allows members the opportunity to remain viable and active and thereby providing their own purpose within their community.

East Shore Regional Adult Day Center, Inc.DBA Orchard House Medical Adult Day Center
 Branford, East Haven, Guilford, Hamden, New Haven, North Branford, North Haven, and West Haven
Therapeutic programs offered at Orchard House include, but are not limited to: expressive art therapy, music appreciation, meditation and reiki, Food with Friends, a shared experience cooking club, computer instructions, pet therapy, a horticultural therapy program, Tai Chi, and arts & crafts.

The Mary Wade Home, Inc., The Mary Wade Home Adult Day
 Branford, East Haven, Hamden, Milford, New Haven, North Haven, West Haven
Operates seven days a week and utilizes a medical model.  ADC clients receive medical evaluation, monitoring and have the opportunity to participate in a wide range of recreational activities that include exercise, crafts, educational and intergenerational programs, and the "It's Never 2 Late (IN2L) technology system.  Clients are provided with nutritious breakfasts, lunches, and snacks. Additionally, Mary Wade provides an enhanced program of daily activities to cater to levels of cognitive limitation.  Every ADC client also has access to additional services offered at Mary Wade, such as physical occupational and speech therapy; and beauty and barbershop services.

In-Home Services

Communication Advocacy Network, Deaf Elderly Services
Ansonia, Bethany, Branford, Derby, East Haven, Guilford, Hamden, Madison, Meriden, Milford, New Haven, North Branford, North Haven, Orange, Oxford, Seymour, Shelton, Wallingford, West Haven, Woodbridge
CAN matches deaf elderly client's with CSP's to assist them with home repair, chore services shopping services or homemaking services.  The CSP's are also deaf and communicate through ASL (American Sign Language).  The visits also serve as companionship for the often isolated deaf elderly.

Franciscan Family Care Center, Inc., The Promoting Independence of Elders
Meriden and Wallingford
The Promoting Independence of Elders Program (PIE) is designed to support vulnerable low income  and minority older adults and to assist them in maintaining their independence to live in their own home.  The program reaches out to isolated elders and provides monthly friendly visits.

TEAM, Inc., Homemaking  
 Ansonia, Derby, Milford, Orange, Oxford, Seymour, Shelton and Woodbridge
 Services provided include: light housekeeping, help with grocery shopping, laundry and running            errands.

 Legal

New Haven Legal Assistance Association, Inc.

To educate the elderly community about their legal rights in the form of outreach and Know Your Rights Presentations. To provide legal representation that achieves long lasting improvement or avoidance of deterioration in the legal rights and living conditions of the elderly community.  To provide referral to attorneys at either no cost, if the client is eligible for legal services, or at a reasonable cost for those well about the normal income and guidelines.
           
Connecticut Veteran Legal Center, Removing Legal Barriers

Connecticut Veterans Legal Center will provide free legal representation to veterans aged 60 and over who are income qualified (veterans who earn less than 150% of the federal poverty guideline).

Disease Prevention and Health Promotion

Stamford Evidence based Chronic Disease Self-Management Programs & Diabetes Self Management Programs (Live Well) for seniors of the AOASCC region. Focuses on the frail elderly and minorities. Dealing with frustration, fatigue, pain, stress and isolation. How to communicate effectively with health professionals, family and friends. Making informed treatment decisions.

Family Caregiver Support
