Community Programs Funded by AASCC, October 2017- September 2018

The following agencies receive grants from AASCC through Federal Older Americans Act funding to provide services for individuals 60 years of age or older.

 Transportation

Meriden Senior Center, Medical Transportation
Area Served: Meriden and Wallingford
Service Provided: Rides to medical appointments Monday through Friday.

TEAM, Inc., Medical Transportation
Area Served: Ansonia, Derby, Seymour, Shelton
Service Provided: Rides to medical appointments Monday through Friday.
Services are subcontracted to Valley Transit District.

Mary Wade Medical Transportation Program
Area Served: East Haven, Hamden, New Haven, West Haven and Woodbridge
Service Provided: Rides to medical appointments Monday through Friday.

Mary Wade, Weekend Transportation Program
Area Served: East Haven, Hamden, New Haven, West Haven and Woodbridge
Service Provided: Week-end rides for shopping, social gatherings, church and other activities.

Community Support Services

BH Care, Inc., Elder Abuse Education Services
Area Served: Greater New Haven
Service Provided: Education to older adults and their caregivers on domestic violence at locations such as senior centers and housing sites.

Community Health Center, Inc., CHCI Mericares Senior Dental Program
Area Served: Branford, East Haven, Guilford, Hamden, Madison, Meriden, Milford, New Haven, North Branford, North Haven, Orange, Wallingford, West Haven and Woodbridge
Service Provided: Routine prevention, restorative services and emergency care

Bridges...A Community Support System, Inc., Counseling and Community Support Services for Older Adults
Area Served: Milford, Orange and West Haven
Service Provided: Mental health and/or substance abuse services.

Fair Haven Community Health Clinic, Inc., Elderly Health Program
Area Served, New Haven.
Service Provided: FHCHC provides health information & referral and health assessments/screenings, dental services and mental health services.

 New Haven Legal Assistance Association, Inc. Elder Law Unit
Area Served: South Central Connecticut
Service Provided: Legal services including advice, brief service, full representation, policy advocacy and community education. LAA will provide services for free or will make a referral to an attorney who will provide services for free or at a low cost.

In-Home Services

Casa Otoñal, Inc., Quality of Life Improvement for Latino Elderly
Area Served: New Haven
Service Provided: Language translation services to reduce barriers in communication and increase access to services for seniors with limited English proficiency. 

Communication Advocacy Network, Deaf Elderly Services
Area Served: Ansonia, Bethany, Branford, Derby, East Haven, Guilford, Hamden, Madison, Meriden, Milford, New Haven, North Branford, North Haven, Orange, Oxford, Seymour, Shelton, Wallingford, West Haven, Woodbridge
Service Provided: Utilizing a pool of trained providers fluent in American Sign Language (ASL), the program is equipped to offer home repair, chore services, shopping services or homemaking services. 

 Franciscan Family Care Center, Inc., The Promoting Independence for Elders Program (The PIE Program)
Area Served: Meriden and Wallingford
Service Provided: Personal, in-home visits, which help older adults remain safely at home by assessing needs and providing needed support.

Interfaith Volunteer Care Givers
Area Served: South Central Connecticut
Service Provided: Weekly in-home visits by volunteers providing companionship. Other services might include light chores, assistance with grocery shopping and escorted rides to medical appointments.

TEAM, Inc., Homemaking
Area Served: Ansonia, Derby, Milford, Orange, Oxford, Seymour, Shelton and Woodbridge
Service Provided: Light housekeeping and shopping

Health Promotion/Disease Prevention/Medications Management

Griffin Hospital, Chronic Disease Self-Management Program
Area Served: Ansonia, Derby, Meriden,,New Haven, Orange, Oxford, Seymour, Shelton, Woodbridge
Service Provided:  Through a six-week workshop series, this program teaches techniques that helps individuals live with chronic illnesses.

Griffin Hospital, Evidence Based CDC Diabetes Prevention Program
Area Served: Ansonia, Derby, Meriden,,New Haven, Orange, Oxford, Seymour, Shelton, Woodbridge Service Provided: Year-long program addressing the multiple factors that contribute to diabetes with the goal of preventing the occurrence of diabetes through diet and meal planning, exercise introduction, lifestyle modification.

Grandparents Raising Grandchildren

Christian Tabernacle Church

Service Provided: Respite opportunities for grandchildren and grandparent caregivers; monthly support group and phone support.
